eVantageHealth Privacy Policy

End User Privacy Statement

Introduction

eVantageHealth (the "Service") is a health management tool from eVantageHealth for individuals to collect, store, and update their personal health information. In this document, "We" or "our" refers to eVantageHealth, LLC. The Service is powered by St. Luke’s University Health Network. It is important for you to know how the information that you submit, review and edit in the Service is handled while you use this site.

Collection of Personal and Usage Information

You enter data in the Service when you use the Service, and when you use some of the Service's tools and features. That data is stored in eVantageHealth in your eVantageHealth account. The Service does not permanently store any of your data. The Service uses information you authorize the Service to access from your eVantageHealth account to provide you with personalized health information. You can review and edit information from your account in the Service, and any changes will be saved to your eVantageHealth account. All of this information, when it is held by or transmitted to the Service outside of eVantageHealth, is referred to as "Personal Information."

We may collect information about your interaction with the Service. For example, we may use website analytics tools on the Service to retrieve information from your browser, including the site you came from, the search engine(s) and the keywords you used to find the Service, the pages you view within the Service, and your browser's width and height. We also may use technologies, such as cookies and web beacons (described below), to collect information about the pages you view, the links you click and other actions you take on the Service. Additionally, we collect certain standard information that your browser sends to every website you visit, such as your IP address, browser type and language, access times, and referring website addresses. This information is referred to as "Usage Information."

How your Personal Information is stored

The Service temporarily stores or "caches" Personal Information as necessary to provide you the Service. The Service does not permanently store any of your Personal Information.

All of the Personal Information you enter, edit or view in the Service is held only temporarily by the Service. Information you enter or edit in the Service is added to your eVantageHealth account once you hit "Save.". The Service can also pull the information you authorize from your eVantageHealth account. You cannot use the Service unless you have an eVantageHealth account.

When you hit the "Save" button, the cache of information used by the Service is also updated with the new information you added or changed. Some webpages may also have a link to refresh the cache. When you click that link, the Service updates all the information in the cache with the most current information in your eVantageHealth account.


If you do not already have an eVantageHealth account, then you will be directed to an account creation page where you can create an account. After completing the account creation page, you will be sent an invitation email with an invitation code that you will use to activate your eVantageHealth account before logging in. You should carefully review the eVantageHealth privacy statement and terms of use to understand how eVantageHealth collects, stores and uses your personal information. All Personal Information that you enter through the Service is encrypted using Secure Socket Layer technology to help protect it while it is sent over the Internet. Personal Information and Usage Information are stored on servers located in the United States.

How your Personal Information and Usage Information are used and shared

Your Personal Information and Usage Information are used to provide you the Service and to personalize your user experience. We and our suppliers may also use this information to improve our respective products and services, including this Service. We may engage other companies to provide services under contract, such as hosting and supporting the operation of the Service. We allow those companies to only access the Personal Information and personally identifiable Usage Information that they need to deliver the Service, such as IP address or e-mail address. Except as set forth in this statement, we will not share your Personal Information and Usage Information in personally identifiable form with any other party without your consent, unless we are required to do so to comply with the law, or to prevent report or investigate illegal activity.

It is, however, important that you understand that the choices you select in eVantageHealth may allow other people to access your personal information that is stored in your eVantageHealth account. Make sure you carefully review the privacy statement and terms of use for eVantageHealth so that you understand how your information will be stored and shared.

Third party links

The Service may contain links to websites operated by third parties. We have no control over the privacy policies and practices of such third party sites, and you should review the privacy policies and terms of use of those sites for more information about the policies applicable to those sites.

How we use cookies

"Cookies" are small pieces of information that some websites use when you visit them to store information on your computer's hard drive. Like many other websites, the Service uses cookies to provide information relating to the sources of site traffic and to help you personalize your experience. Our collection of this information, however, does not allow us to personally identify you.

Most web browsers automatically accept cookies but allow you to modify security settings so that you can approve or reject cookies on a case-by-case basis. If you choose to decline cookies, you may not be able to sign in or use other interactive features of sites and services that depend on cookies, and some advertising preferences that are dependent on cookies may not be able to be respected.

 

Complaint process

If you have a complaint or problem related to the Service, you may email us from Website Support after clicking the "Help" link on any page. Based on the nature of your complaint, it will be directed to the appropriate Customer Service department at eVantageHealth by transfers to the appropriate party's support site and may be shared with each other. If you don't receive adequate resolution of a privacy-related problem, you may write to the Service's Security and Privacy Officer at:

If you've contacted the Service's Security and Privacy Officer about a privacy-related concern and you do not believe your problem has been addressed, you may file a complaint with the eVantageHealth Chief Security Officer by calling the eVantageHealth Support general number at (484)526-8893 and ask for the Chief Security Officer.

Changes to our Privacy Statement

We will occasionally update this Privacy Statement. If we do, you will be asked to accept the new privacy statement before you use the Service, the next time you sign in.
